New and updated for InDesign CS5.5.
This book as based on this WIKI article here and the screencast here. I provide this as an example. I will probably update this sometime, although I did a lot of handcrafting the innards of the ePUB.
InDesign CS5.5 has some significant improvements for making ePUBs. Hopefully this little book will explain the basics.

Technical Stuff
I started out using Apple's Pages, and - indeed the PDF was quite successful. However, I found Pages to be very unreliable when it comes to the export for ePUB. This may be improved in future versions, but I decided to go for InDesign, since this is what I am talking about in the book!
I have made a few edits to the CSS and XHTML inside the ePUB. For this I used OxygenXML editor.
